There could be difference of opinion on which is Isme Azam - Allah knows best
I found these so far =

# "Al-Hayyul Qayyoom"
http://salaf-us-saalih.com/2010/05/30/invoking-allaah-by-his-greatest-name-and-its-virtue/
http://www.quran4u.com/Tafsir%20Ibn%20Kathir/002%20Baqarah%20II.htm

# Another hadith on Ism e Azam.  Just find the words "Ism e Azam"
http://www.quran4u.com/Tafsir%20Ibn%20Kathir/112%20Ikhlas.htm
(It looks like a mixture of Kalima Taiyyab's 1st part and Sura Ikhlas)

# Some people say its in Dua-e-Yunus/Ayat-e-Kareema  as well as Sura Ikhlas (reference not provided)
http://ashraf786.proboards.com/thread/28723

# Sura Fatiha also has "Greatest Name" (but its not called here Ism-a-Azam)
http://www.quran4u.com/Tafsir%20Ibn%20Kathir/001%20Fatihah.htm

# These sites are in probably Urdu or Arabic. They show Isme Azam can be calculated based on user's names, circumstances, etc:

"Al-Hayyul Qayyoom"

Imam Ahmad recorded that Asma' bint Yazid bin As-Sakan said,

"I heard the Messenger of Allah say about these two Ayat, اللّهُ لاَ إِلَـهَ إِلاَّ هُوَ الْحَيُّ الْقَيُّومُ  Allah! None has the right to be worshipped but He, the Ever Living, the One Who sustains and protects all that exists) (2:255), and, الم - اللّهُ لا إِلَـهَ إِلاَّ هُوَ الْحَيُّ الْقَيُّومُ  (Alif-Lam-Mim. Allah! None has the right to be worshipped but He, the Ever Living, the One Who sustains and protects all that exists), (3:1-2)

They contain Allah's Greatest Name.

This is also the narration collected by Abu Dawud, At-Tirmidhi and Ibn Majah, and At-Tirmidhi said, "Hasan Sahih''.

Further, Ibn Marduwyah recorded that Abu Umamah reported that the Prophet said,

Allah's Greatest Name, if He was supplicated with it, He answers the supplication, is in three Surahs - Al-Baqarah, Al Imran and Ta-Ha.

Hisham bin Ammar, the Khatib (orator) of Damascus (one of the narrators in the above narration), said,

"As for Al-Baqarah, it is in,

Allah! None has the right to be worshipped but He, the Ever Living, the One Who sustains and protects all that exists,

in Al Imran, it is in,

Alif-Lam-Mim. Allah! None has the right to be worshipped but He, the Ever Living, the One Who sustains and protects all that exists, (3:1-2),

while in Ta-Ha, it is in,

And (all) faces shall be humbled before (Allah), the Ever Living, the One Who sustains and protects all that exists, (20:111).

In his Book of Tafsir, An-Nasa'i recorded from Abdullah bin Buraydah, who reported from his father that he entered the Masjid with the Messenger of Allah, and there was a man praying and supplicating saying,

"O Allah! Verily, I ask you by my testifying that there is no God worthy of worship except You. You are the One, the Self-Sufficient Sustainer of all, Who does not give birth, nor were You born, and there is none comparable to Him.''

The Prophet said,

By Allah in Whose Hand is my soul, verily he has supplicated with Ism e Azam

. Whenever supplication is made with the greatest name of Allah, it is blessed with acceptance.

Dua Yunus / Ayat e Kareem

It is said that Allah is the Greatest Name of Allah, because it is referred to when describing Allah by the various attributes.

Further, it was reported that

Ar-Rabb is Allah's Greatest Name.
